How do you download and sign the current form?
Short answer: Download IMM 5476 from the official IRCC form page on a desktop. The current PDF is version 11-2025. Save it, then open it in Adobe Acrobat Reader 10 or higher. Do not use a phone, a browser PDF viewer, or a commercial form-filling site. Those copies are often years out of date.
If you submit through a portal or secure account, IRCC accepts a wet (hand) signature, an e-signature, or a thumbprint if the person cannot make a mark. E-signature includes a typed name, Adobe Fill & Sign or DocuSign, a scan or image of a signature, or a check box as indicated. If you submit outside a portal or secure account, IRCC wants a wet signature. A scanned wet signature is acceptable. A thumbprint is acceptable if the person is illiterate or cannot make a mark for a physical reason.
Download the PDF from that IRCC page. The current file is version 01-11-2025. That is the file to keep.
How do you submit IMM 5476, including after you have already applied?
Short answer: If the immigration or citizenship application is not in yet, upload IMM 5476 with the online package, or send it with a paper package to the address in that application's guide. If the application is already in, submit the form through the IRCC web form, or mail it to the office that is processing the file if you know the address. You must notify IRCC if the representative's contact information changes, if you change representatives, or if you cancel the appointment.
The web form IRCC points to is Client Contact form 71. Use it for a form that is late to a file that is already in processing. Do not assume a portal upload is still open after you hit submit on the main application.
Keep a copy of what you sent. If you appoint someone and then hear nothing, the first check is whether correspondence is landing in the representative's inbox instead of yours.
What gets an application returned?
Short answer: Naming a paid representative who is not in good standing. Using a pre-11-2025 PDF after 12 March 2026. Appointing two people on one application. Completing the wrong sections. Skipping the signatures in section E. Using IMM 5475 when you needed 5476, or the reverse. Forgetting that an 18-year-old dependant needs their own form. Those are paperwork errors. IRCC can return the whole application for them.
Before you name anyone, confirm CICC, law-society, or Chambre standing yourself. Then be clear about what you are signing. IMM 5476 redirects correspondence and hands over the authority to act. If all you wanted was a status check, that is IMM 5475.
